1. Fill out(please include full first name, middle initial and last name), Sign, Date and Print 2 copies of the Online Contract. Make sure your printer is set to print this on one page. Send both copies to A-ALERT DRIVING SCHOOL, LLC 1116 WALNUT STREET, WEST BEND WI 53095. **The Student will not be enrolled until we receive these.
2. Be sure to include payment with your contract. We will accept an online payment via PayPal, a Money Order, or check. Please specify payment type on the contract by checking the appropriate "Payment Included" box. Make sure to put beginning class date and location at top of contract (for dates go to classes offered )
3. To pay online,click on the appropriate pay now button below, this will take you to paypal. Also if using a business account to pay please include your name in paypal, so we know who the payment belongs to.
